Training and Development The role includes an immersive 12‑week training program of daily live instruction, interactive learning modules, role plays, and practice with real patients, followed by a two‑month apprenticeship with experienced coaches. What to Expect Full‑time commitment: 8 hours per day during training, with two days per week in‑office. Hands‑on practice: frequent assignments, role plays, and feedback to build confidence and skill. Real‑world preparation: learn Motivational Interviewing, care coordination, and how to help patients navigate social needs such as food, housing, and transportation. Culture of support: dedicated trainers, clinical mentors, and peer cohorts guide you every step of the way. What You’ll Do As a Health Coach, you will partner with patients to build healthy habits, overcome barriers, and connect them with appropriate resources, while coordinating care with a multidisciplinary team. Key Responsibilities Conduct one‑on‑one coaching sessions in homes or community settings with up to 20 patients weekly. Educate patients about chronic conditions including diabetes, COPD, asthma, CHF, and hypertension. Motivate and empower patients through goal setting, accountability, and culturally sensitive coaching. Help patients engage or re‑engage with primary care, schedule visits, and ensure follow‑up after hospitalizations. Support care coordination by reviewing charts, confirming appointments, and preparing patients for visits. Guide patients to community, health plan, and government resources using directories and tools. Document patient encounters and progress in the internal Salesforce database. Collaborate with Nurse Practitioners and Social Workers on case reviews. Communicate patient risks or complications to the clinical team promptly. Assist with additional tasks as needed to meet patient and team needs. What You’ll Bring At least 2 years of experience in customer service, community work, teaching, case management, or other people‑centered roles. High school diploma or GED required (some college or healthcare experience preferred). Strong interpersonal and organizational skills; ability to build rapport quickly and follow through. Comfort with technology and adaptability; experience with Salesforce or willingness to learn. Ability to work independently and collaboratively across a care team. Commitment to health, wellness, and supporting others in reaching their goals. Local travel within the Queens community (up to 70 %) required. In‑Person Training & Work Requirements This role requires in‑person training and patient visits. Timeline & Compensation Anticipated start date: February 2026. Hourly rate: $26.00. Annual performance‑based bonus: contingent on individual contributions and company goals.
